If you are a new registrant to our program, please answer the following questions. The answers you provide will pre-register you to be considered for our upcoming clinical studies. (Note: This survey is NOT designed to determine specific study eligibility. If you need to update your contact information or prescription, please email or call us.)
1. Can you visit the clinic in Johns Creek, Georgia Monday through Friday during regular business hours? *This question is required.
2. Do you currently wear soft contact lenses that correct for myopia and/or astigmatism? *This question is required.
3. Are you over 18 years of age? (If your child is under 18, please contact us directly. Do not use this form.) *This question is required.
4. Have you had Refractive/Lasik eye surgery? *This question is required.
5. Have you had any eye surgery within the previous 12 months? *This question is required.
6. Do you have any clinically significant eye disease? *This question is required.
